چکیده

Malaria is a protozoan disease caused by the bite of a female anopheline mosquito which is infected by the protozoa Plasmodium. It is the world’s third-ranked infectious killer disease after HIV/AIDS and tuberculosis1. Currently, six species of Plasmodium (P. falciparum, P. vivax, two sympatric species of P. ovale, P. malariae, and P. knowlesi) are known to cause malaria2. Of these, P. vivax was responsible for 13.8 million cases of malaria in 2015, with most cases (76%) occurring in the Southeast Asia Region (SEAR) where India and Pakistan share the burden3. Previously, complicated malaria was considered to be exclusively due to P. falciparum. However, over the recent years, reports from different parts of world have shown that P. vivax has similar if not equal potential to cause complications. Multiple cases of severe malaria due to P. vivax have been reported with complications, like anaemia, hepatic dysfunction, renal failure, cerebral malaria, ARDS4-7 and retinal haemorrhage8. Various neurological complications have been reported with P. falciparum malaria including cerebral infarct, but such manifestations have been reported intermittently in case of P. vivax9-10. Here, we report a unique case of acute cerebral infarct due to P. vivax malaria in a previously healthy young adult. This case report highlights the atypical presentations of vivax malaria which clinicians should be aware of, while making differential diagnosis in endemic zones as well as hypoendemic zones of malaria.

Malaria has been among the most common infectious diseases in India. The causative microorganism belongs to the Plasmodium family. The four common species that are encountered are Plasmodium falciparum, P. vivax, P. malariae and P. ovale. Plasmodium falciparum infection carries more risk of complications such as cerebral malaria, acute renal failure, liver damage, collapse etc. Plasmodium vivax...

Vivax malaria presenting with myelitis: a rare complication.

Neurological complications may occur with the Plasmodium falciparum infection. However, the association of neurological manifestations with vivax malaria remains doubtful. Of late, there are isolated case reports/studies which have implicated P. vivax in the pathogenesis of severe malaria which is characterized by the features of different organ dysfunctions, which were previously thought to be...

Cerebral malaria caused by Plasmodium vivax in adult subjects

Cerebral malaria is a diffuse encephalopathy associated with seizures and status epilepticus which can occur in up to one-third of patients with severe malaria, particularly that caused by Plasmodium falciparum. In this article, we report three cases of Plasmodium vivax malaria (all adult male patients) complicated by seizures and symptoms of diffuse meningoencephalitis. Two patients had predom...

Acute attack of AIP (acute intermittent porphyria) with severe vivax malaria associated with convulsions: a case report.

Acute intermittent porphyria (AIP) is a hereditary hepatic porphyria inherited as autosomal dominant with low penetrance resulting from mutation in the gene encoding the enzyme, porphobilinogen deaminase (PBG-D) [hydroxymethylbilane synthase, uroporphyrinogen I synthase]. The diagnosis is often obscured by the variable presentation, viz. abdominal pain (commonest), constipation, vomiting, convu...
